If an agent attaches an image to an issue and embed it to a comment, then the notification of this comment will show and error icon instead of the image.
Steps to reproduce
- Attach image to an issue
- Send a comment including the image
e.g. - Check notification e-mail in order to see the comment and the image

This issue should be resolved on the latest JSD for Server and Cloud, in relation to
JSD-1370.There is still an open bug for JIRA Core, on
JRA-41675. May I ask if these emails in question are the JSD customer notifications? If JSD notifications are turned off, or the notifications are sent to JIRA users not involved as customers on the ticket, then the JIRA notifications will be used andJRA-41675may be the issue you are facing.To help understand the problem, could you create a support ticket at https://support.atlassian.com?
