Description
Summary
If a Portal Only user is logged in, all Issue Collector fields other than "Name" and "Email" will not show breaking all usability of the collector.
Affects

Steps to Reproduce
- Create an issue collector using the Custom Template
- View the issue collector with a non logged in user. e.g: in incognito mode
- Log in with a Portal Only user
- View the issue collector with that user
Expected Results
Issue collector shows normally because the Issue Collector is public
Actual Results
Only "Name" and "Email" fields show for the Portal Only user but all fields configured show for public users.
Notes
This does not affect the templates "Got Feedback?" and "Raise a bug" that show all fields regardless the user session.
Workaround
Log out the Portal Only user to create the request.
Optional until bug is addressed: add a mention to the "Message" of the collector to warn Portal Only users of the problem so they logout. e.g: "If this form is only showing "Name" and "Email" please logout from <site-url> and refresh the page."
