- define a request type in a customer portal
- add a field (for instance 'Description') and set a custom 'Display name' (for instance ('Any additional details you can provide?')
- go to the customer portal
- raise a request
- go to the JIRA issue
- it would be convenient, from a customer service team member point of view, to know the exact question the customer was asked next to this field. This could be done by adding display name next to the field name. In this case:
