      As a Service Desk Administrator, I would like to add a form as an attachment, where I would need my customer to fill in, and attach it back into the ticket.

      Having a read-only Custom Field URL field or this functionality itself into the Request Form will help me to achieve this use case.

            Such a shame it is closed, I was looking for this feature.

            FYI right now I am using the link and it is working well. If anybody is curious, the format is as below, You put it in the instructions filed and it will show the link name which is clickable.

            This is easy to do if you put a link in the Field help. I normally use bit.ly links here.

            The url should be a part of the Request Type form?

            I imagine that you would like a label above the url, or simply a url where you specify the link and text?

            Something similar to this should be possible at the moment, as you can add a link to the description of existing fields in your request type.

            We will generate an internal website to accommodate forms available for the users and add the url in the request form information.
