As a Service Desk Administrator, I would like to add a form as an attachment, where I would need my customer to fill in, and attach it back into the ticket.
Having a read-only Custom Field URL field or this functionality itself into the Request Form will help me to achieve this use case.

Such a shame it is closed, I was looking for this feature.
