Display fields in Customer Requests in read-only mode

      It would be useful to be able to add fields to the Customer Request forms with a read-only option - they would be displaying information to the customer, but couldn't be changed by them.

      This would allow the Agents to communicate information through Issue Fields (e.g. Fix Version).
