[FORMDC-61] Add additional forms based on previous fields filling

Ability to add additional forms based on previous field filling.

External: dynamic add a form on the Portal(like a "+" to add an additional form)
Internal: add a form using automation
Attach a form based on a Jira field value.

Option 1: Create a conditional logic for dynamic fields to add some options when you select a specific item.
