duplicate issues created when clicking on create button multiple times

      A continuous clicking of "Create" button in Customer Portal creates as many issues as there were clicks.

      This is undesirable behaviour.

            Submit button is not disabled in cv-request-create-view.js onSubmit function. Please add that call and corresponding enableSubmit as well.
