      At this time, it is not possible for customers to determine who executed on actions related to forms.

      Suggestion

      It would benefit administrators to have additional tracking information per issue history or platform logging in order to audit these actions.

      Use Case:

      In the Issue View, when forms are added, submitted, or deleted there are no actions noted in the issue history that would allow auditing these actions.

      Workaround:

      Linking custom fields to form fields will update the issue history when the form is submitted, which gives an indication of who updated those fields through a form. However as fields may also be updated directly, this information may not be definitive.
