Summary
In JSM Forms is currently not possible to assign a form request to an agent based on a field value/response in the portal.
Workaround
Currently, we have a workaround using Automation for Jira(A4J) or Forms API to assign a form based on a field response, but only after the ticket has already been created on the portal:
