History should be set when each approver approves a transition

Problem Definition

When there are multiple required approvers added in a transition, Issue History and Activity is not updated until all approvers have approved the transition. It would be useful to have a history item set for each of the approves, so that the users would know when each approver has approved the issue for auditing purposes

Suggested Solution

JSD should log each approvers action as a system event, and log a history event for each action applied by the individual approver, So that when the approval action is completed per user a timestamp is included for review and audit.
