Create issue carries over attachment from previous issue

      Summary

      When performed in a certain way the menu where new issues are created uses the attachment from the previous created issue.
      We are using the workaround which is provided in JRA-34926. We are working in a Call center environment with a productive Jira 6.1.4 instance and we do need this bug to be removed while using the workaround with the snippet in the Announcement banner (see page https://confluence.atlassian.com/display/JIRAKB/Disable+the+new+Issue+Creation+dialog+in+JIRA+5.0). Otherwise our user are not able to do their work properly!
