Creating issues using direct link remembers the attachments from previous issues

      Steps to reproduce :

      1. Try to create a issue using a direct url like : http://localhost:8604/secure/CreateIssueDetails!init.jspa?pid=10000&issuetype=1&components=10000&priority=1
      2. Add attachments to the issue and click Create to submit the form.
      3. Now try to create another issue by going to the same url : http://localhost:8604/secure/CreateIssueDetails!init.jspa?pid=10000&issuetype=1&components=10000&priority=1
      4. Notice that the attachments are still there on the page
      5. The browser caches are still storing the attachments.
