Issue Summary

When accessing the Action Item Report it is not being possible to add a new item using the button "Add New". The slide pane remains blank.

This issue is happening on new and old navigation UI. 

Steps to Reproduce

1. With the new UI enabled, go to* Items -> Objectives -> More Action -> Actions Item Report. *
2. Click on the "Add New" Button.
3. Check the slide pane on the right side.

Expected Results

The page loads the slide pane screen related to the "Add New" button when clicked.

Actual Results

The page remains blank.

Inspecting the page, we are able to see an error 500 - Internal server over the URL:

Workaround

Currently, there is no known workaround for this behavior. A workaround will be added here when available
