Issue Summary

When using date picker fields in forms, we can change their format by selecting a different language in the Form setting(i.e. from DD/MM/YY to MM/DD/YY), while the date format looks correct in the Form, however, once the Form is submitted the date is inconsistent in the PDF, it will always set the date format to DD/MM/YY no matter the language used.

Steps to Reproduce

• Create a Form with a date picker question;
• Set the Form Language to English (US)
• Set the Submit behavior to Lock the Form and attach a PDF.

Expected Results

The date format should be consistent in the Form and in the PDT attached (MM/DD/YY)

The date format is set to DD/MM/YY:

No workaround is available, the date format is hard coded.
