We use Crucible with Subversion to perform post-commit reviews.

If we add two Subversion changesets/revisions to a single review, the first which creates a file and the second which modifies that same file, then Crucible only shows the file changes for review, it doesn't show the file addition.

This is problematic because it means that some changes may sneak through unreviewed.

As a result, our reporting forces the use of separate reviews for file additions, which is a source of difficulty for our users. It's particularly unpleasant when a file addition must only be approved with subsequent modifications, but the tool requires that approval to be separated across reviews.
