Refactoring doesn't change pages when they're in a macro

    Description

      We need to add some kind of "refactorable" interface to macros, so that they can intelligently rename themselves when they refer to pages.

      Affected macros include:

      {excerpt-include} {include} {children}

      (when listing another page)
