Issue Summary
Hello team,
Noticing issues with attaching files to the Incident postmortem draft intermittently to only with few accounts. I just see a broken image when I try to attach anything without any errors. However, in the Inspect Network details can see a 401 error with "abc@email.com
does not have access to this domain." Same error with users with Owner access too.
This is reproducible on Data Center: (yes) / (no)

Steps to Reproduce
- Create an incident postmortem
- From the postmortem draft just edit the content and try to attach anything. Please make sure to attach from within the file edit and not from the + Add attachment section as this also gives the error but after a few page refreshes it is available.
- When from within the content we try to attach the file gets attached but gets the broken image

Expected Results
Get the file attached and display it in the file body content.
Actual Results
Getting a broken image and also gives 401 errors
Workaround
Currently, there is no known workaround for this behaviour. A workaround will be added here when available
