Issue Summary

This is reproducible on Data Center: yes

Steps to Reproduce

1. Access or create an Advanced Roadmap Plan
2. Click on Give Feedback
3. Fill out the feedback information and click on Submit

Expected Results

A feedback is properly created in JAC.

Actual Results

The below error is displayed:

"Oops! Something went wrong.... There was a problem submitting your feedback, likely due to the configuration of this form. You might want to contact the site owner to let them know about this issue."

While collecting a HAR file, you can see a 404 error for a post request to JAC:

It works properly on Firefox
