Field Templates inside Customer Portal

      As a service desk user I require the ability to have a template provided on the Customer Portal page so I can easily define requirements or information that should be submitted before submitting the Customer Request.

      These templates should be possible to define per Customer Request type and fields such as Summary and Description should be possible to define a "template" of what information the user needs to provide before submitting the request.

      See attached screenshot mockup of example. This is a standard function in majority of ITSM or form based request systems today.
