Currently "Comment required when approving" or "Comment required when declining" checkboxes if checked then a comment screen is provided to the approver to add additional notes during the approval process but if it is not checked then there is no comment box available.

Basically there is no way to get the comment box if the comment is optional, it is only displayed when the comment is required.
