Issue Summary

Request form description reverts to default when the Request type name is modified.

Steps to Reproduce

1. Go to Project settings > Request types > Select a request type.
2. On the Request form, change the Request form description.
3. Edit the Request type name.

Expected Results

The Request type name will be modified without affecting other fields/details.

Actual Results

The Request form description is reverted to its original description.

Workaround

Change the Request form description after changing the Request type name.
