Issue:
Due to bad data accumulated in the backend in a form (proforma), there are errors evident while submitting a form on the portal like "The 'x' choice is required", even tho the field isn't linked to any Jira field or doesn't have any required response configured.

Workaround:

To clear the bad data, you need to select and de-select and add and remove the related options in the "Required response" field in the form.
