Current State:

Currently when a form is attached to a work item and is downloaded, it does not show any headers.

Proposed Future State:

At a minimum, all forms' PDF headers should show:

• Issue key

• Issue summary

• Request type name

• Customer name

• Organization of the customer

• Date submitted

This will help to gain exact information from the forms.
