Forms can override "Attach to request types" settings

      Issue Summary

      FORM from the Request Type gets removed , if Request Type is opened in one tab and FORM is in another tab. 

      Steps to Reproduce

      1. In one browser tab, open Request Types and create a new request type
      2. In another browser tab, open Forms and create a new form
      3. In the Request Types tab, attach the Form to the Request Type
      4. In the Forms tab, make any changes to the Form and save

      Expected Results

      The Forms remains attached to the Request Type

      Actual Results

      the Form is removed to the Request Type. This is because the "Attach to request types" toggle is OFF, since the form was attached in a separate browser tab. Saving the changes will override this setting.

      Workaround

      Refresh the Forms page before making any further changes to the Form and make sure the "Attach to request types" toggle is ON.
