Issue Summary
The "Create work item with a request type" automation action requires field to be added on Request Type form as detailed here:
Available fields include all fields that are not hidden and added to the request type. Hidden fields aren’t supported, except for Major Incident, which is available for Premium or Enterprise plans.
However, it also does not allow to use fields attached to the request type through JSM Forms.
Workaround
Add desired fields to Request Type request form, not JSM Form, then they will display on the rule action.
