Sometimes the Form is missing after the request is created

      Issue Summary

      Sometimes, when we create a request from the JSM customer portal, the request type has a form loaded on the create request screen. After creating the request, the form is not attached to the request.

      This is not a standard behaviour and only occurs when this bug hits.

      Steps to Reproduce

      1. Go to JSM project settings and attach a request type to a JSM form
      2. Go to the JSM customer portal and submit the request, including the JSM form
      3. After the request is created, check the request
      4. JSM Form is not attached to the request

      Expected Results

      Form is attached to the created request.

      Actual Results

      Form is not attached to the request.
