Issue Summary
Sometimes, when we create a request from the JSM customer portal, the request type has a form loaded on the create request screen. After creating the request, the form is not attached to the request.
This is not a standard behaviour and only occurs when this bug hits.
Steps to Reproduce
- Go to JSM project settings and attach a request type to a JSM form
- Go to the JSM customer portal and submit the request, including the JSM form
- After the request is created, check the request
- JSM Form is not attached to the request
Expected Results
Form is attached to the created request.
Actual Results
Form is not attached to the request.
