Issue Summary

If a JSM form is associated with a request type, it is possible that the page loads, but the form does not, due to unreliability in the customer's Jira database. 
In some situations, if the form has required fields and this bug is triggered, the request can be submitted without those fields being filled in

Steps to Reproduce

It is an intermittent issue that is dependent on the load on the tenant Jira database.

Expected Results

If JSM form load is unavailable due to Jira unavailability, then Portal create page should not load or be able to be submitted without a form.

Actual Results

The Portal create page loads but the JSM form does not.

Workaround

Currently there is no known workaround for this behavior. A workaround will be added here when available
