Issue Description

The form fields such as “Raise this request on behalf of” and “Summary” are marked as required with “*”. Although an asterisk is provided for the mandatory fields, the instruction stating the meaning of the asterisk is not provided.

Steps to Reproduce

1. Open “Raise a Request” page.
2. Navigate to form fields and observe screen reader announcement.

Actual Behavior

An asterik is provided for form fields but the instruction stating the meaning of the asterisk “*” is not provided. Screen reader users often prefer identifying mandatory form field information in advance to complete the form successfully and avoid error messages. Well-defined mandatory form fields are also helpful for users with mobility impairments as they can fill up the form with ease. In the absence of mandatory form field information, users might incorrectly fill up the form and thus might lead to frustration.

Expected Result

Provide the instruction explaining the meaning of the asterisk such as “*” denotes mandatory fields”. Place the instruction at the top of the form to ensure all users can access the instructions.
