Show Customer DisplayNames in Request Type Form in Assist/Slack

      Issue Summary

      Assist/JSM Chat shows the Issue field names, rather than Customer Display Names.

      This is reproducible on Data Center: (yes) 

      Steps to Reproduce

      1. Set Display Names on the Request Type
      2. Sync Request type to JSM Chat
      3. Create a ticket in Slack
      4. Form shows Issue Field Name

      Expected Results

      Form should show Customer Display Name. 

      Workaround

      Change the name of the Issue Field Name to the Display Name
