Forms User Picker Field: Improve the user picker field to display avatar and email address of users

Problem Definition

Currently, the forms user picker field just displays the usernames of users. This would work fine when they aren't users with the same username.

When two users have the same name, it ends up confusing the issue creator on the right one to choose.

A workaround would be to use the Jira native fields but some difficulties using the Jira native field are:

• You can't place the custom field between form fields. It has to be on top of all the forms field because the form is always displayed under the Jira fields.
• You can't write annotations in front of it because this is just a Forms feature.
• You can't place it next to another field (as shown in the screenshot) because this is just a Forms feature.
• You can't display or hide it dynamically depending on another field's value.

Improve the Forms user picker field to show avatar and email address like the Jira field
