    Description

      Add the ability to add a form within a workflow transition screen, as opposed to automatically adding a form during a transition.

      For example, if a user Closes an issue in JIRA, they click the close button and we can make it such that a screen pops up that allows them to put options into the screen before the issue is closed. Right at this point and on this Close screen, we would like to have the Proforma forms show up for this issue.?
