Form Field: Paragraph - support rich text editing

Rich text fields are now the default paragraph fields in forms.

Existing forms have not been upgraded to use rich text fields, as there are too many possible permutations for this to be done automatically. To change a existing form, edit the form and delete the exist paragraph field and insert a new paragraph field. When previewing the form you will see that it has changed to a rich text field.
