Adding two widgets on the same webpage renders the same form

Issue Summary

When adding two distinct widgets on a webpage, it doesn’t show the correct fields.

Steps to Reproduce

1. Configure and save a widget from two different Service Management projects.
2. Add both widgets to a webpage and modify the CSS for one to not override the other.

Expected Results

When customers visit the page, they can see two widgets and create the ticket on the desired one.

Actual Results

When clicking on the widgets, both show the same fields (forms), even though the request types and projects are not the same. 

Workaround

Currently there is no known workaround for this behavior. A workaround will be added here when available
