Issue Summary

Missing dark theme on the give feedback page.

Steps to Reproduce

1. Click on the user avatar in the top right corner
2. Theme > Dark
3. Again, click on the user avatar in the top right corner
4. Go to "Accessibility" 
5. Click the "Give feedback" button

Alternatively

1. Go to secure/AccessibilityPersonalSettings!default.jspa
2. Scroll down and click the "Give Feedback" button

Workaround

Use light theme
