Issue can be created twice

      Some users had been confused by issues in Jira that have no status. These cannot move around the workflow and have to be deleted. After a bit of digging I found thesee issues had identical twins in the database, these twins were ok.

      I found that I could reproduce this by clicking twice on the create button - not sure if this has been reported before with these consequences.
