Workflow db errors: Violation of unique index

A number of users have reported exceptions when:

• Linking 2 issues together
• changing an issue's component
• changing issue priority
• closing an open issue

For example:

Violation of unique index: SYS_PK_JIRAACTION in
statement [INSERT INTO jiraaction (ID, issueid, AUTHOR, actiontype,
actionlevel, actionbody, CREATED, actionnum) VALUES (10833, 10502,
'dhardiker', 'comment', NULL, 'Possibly affected by the results of ABI-52.
However, where this fits in is still a matter for deliberation. As Daniel
suggested, maybe inside the filter element - or maybe inside its own
