Issue Collector fields (Name and Email) are missing upon canceling the current logged in user's session

When the Issue Collector form appears, it will ask for the field's Name and Email if there is no active JIRA session.

When there is an active JIRA user, it will display We've currently got you logged in as System Administrator. This feedback will be created using this user unless this is not you.

If the user clicks on the not you link, it supposes to show the Name and Email fields. However, these fields are missing:
