Summary

When using custom issue collector form template, it just presented with a blank white page and you can also see a blank screen in the Template Preview

Steps to Reproduce

1. Add Issue Collector
2. Select custom for Issue Collector Form Template
3. Submit Issue Collector

Fields are displayed

Shows a blank page

Workaround

Pre requisites: N/A

Steps: The same issue doesn't happen with preview on Edit mode, so that can be used as reference. To test the form, it's possible to use the embedded link on the site page, where the collector is added to.
