Issue Summary

Updating a Theme's name or description field generates a single Audit Log entry, but creates two entries in tblAgileThemeAudit.

This can lead to reporting discrepancies for Enterprise Insights customers who query the [Theme History] table.

Steps to Reproduce

1. Create a Theme in JA
2. Update the Theme's name field and Save
3. Update the Programs field and Save
4. Update the Description and Save

Expected Results

Each field update should create a single entry in tblAgileThemeAudit to match the Audit Log

Actual Results

Name and Description field updates create two entries in tblAgileThemeAudit which do not match the Audit Log

Workaround

Currently there is no known workaround for this behavior. A workaround will be added here when available
