Steps to Reproduce

1. Navigate to teams
2. Check each teams for duplicates.

Expected Results

A user should only be assigned once in a team.

Actual Results

There are cases that a user is duplicated.

Workaround

Currently there is no known workaround for this behavior. A workaround will be added here when available
