Option to include Issue Attributes in ProForma export

      When attempting to export ProForma data with Jira fields, the Status attribute is not present in the exported data as it is not a Jira Field.

      This is confusing for users as the CSV export of the same JQL search used to "export responses with Jira fields" does contain Status information:
