Summary

When creating a JIRA Issue from a review, only the "Project", "Issue Type", "Summary" and "Description" fields are presented.  If there are any fields designated as required in JIRA, issue creation will fail

Environment

JIRA 6 and Crucible 4

Steps to Reproduce

1. Create Project in JIRA
2. Specify a field as "Required"
3. Create a review in Crucible
4. Add a comment in Crucible
5. Click "Create Issue"
6. Fill in the presented fields.
7. Click "Create"

Expected Results

The "Create Issue" screen presents any required fields from the JIRA Project.

Actual Results

No required fields are presented.

Workaround

Remove the "Required" property from all fields in JIRA.
