Description
when a comment is removed, we get an email notification such as:

Comment Removed : wiki : Re: Common Concerns and their Resolutions
Re: Common Concerns and their Resolutions has been removed by James Mortimer (Apr 18, 2007).
========================
However, the name given is the name of the person that originally WROTE the comment, not the person removing the comment.
(also, there is needless repetition here of the comment title, but we'd probably like to have the page name!)
Request that the notification read:
========================
Comment Removed from wiki : Common Concerns and their Resolutions
"Re: Common Concerns and their Resolutions", posted by James Mortimer, has been removed by John Doe (Apr 18, 2007).
========================
where the first line is the page link, and the seond line is the link to the now non-existant comment (as it currently is), and the two names are also hyperlinked.
