Ability to set floating Table of Contents

Problem Definition

The Table of Contents macro is fixed where you place the macro, but it would be helpful to be able to set it to float so it's always visible and so your users can know where they are on the page.

Suggested Solution

Provide the ability to choose to float the Table of Contents macro and indicate where you are on the page.
