Would like the ability to add a custom screen to the approvers transition

After the implementation of JSDSERVER-4789, on the approver transition comments can be added to JSM issues, however this stops short of allowing a customized screen to be applied to this transition. It would be helpful in situations where the approver must enter or update custom fields to be able to configure your own custom screen and have that appear upon the approval transition.

Workflow transition screens should appear on the transition to which they are assigned, regardless of if there is an approval on that transition.

The approval should not bypass any assigned screen, required fields (custom fields or system fields), or any other condition, validator, or post function assigned to that transition.
