I am getting the same commit messages multiple times while raising a PR to any environment

For eg, I merged 1 PR to the dev env with a commit msg
That same commits[changes are again shown in a different branch while merging it to the same environment, even though those changes are already present in the dev env]
This issue/ info is the same across environments
Please help us, as this creates a lot of confusion while reviewing any PRs
