Require a template or form to be filled when changing the scope of a sprint

      Problem Definition

      Anyone who plans to have a scope change to an active Sprint, is shown a warning in JIRA. Stating that this change will affect the scope of the sprint.
      There isn't a way to track the reason or rationale behind the sprint scope change.

      Suggested Solution

      Show the user a template to be filled before going in for a scope change. The template would include information needed to explain why the user is making the change.
