Add Planning Poker functionality

      Add a Planning Poker link to the JIRA ticket page
       
      This will allow teams to start a planning poker session for a given user story directly from within its ticket. The new planning poker page will behave like the site http://www.planitpoker.com. When voting is over, the story points will be filled into the JIRA ticket.
