Problem:

A "sticky" field behaviour is reported on the Jira Create Issue screen. When using the "Issue Templates for Jira" add-on, field values (such as Summary and Description) populated by a template don't clear when the user switches the target Project while the dialog is still open. 

Solution needed:

The template info disappears; meaning that summary and description part of the template should be reset and emptied out.
