Issue Summary
When editing an issue collector, the custom field options that were previously checked appear as unchecked.
Steps to Reproduce
- Create an issue collector
- Select the custom template, and check one or more custom fields (Component/s, Description, Priority, Environment, Affects Version/s, Attach file) to be included in the template and save the changes
- Implement the issue collector and test it; view the form and validate the custom fields that you selected in step 2 are visible as expected
- Edit the issue collector and observe the custom field checkbox of the custom template
Expected Results
Checked custom fields appear as checked.
Actual Results
Checked custom fields appear as unchecked.
However, the custom fields are added into the issue collector successfully

Workaround
Currently there is no known workaround for this behavior. A workaround will be added here when available
